My Role in Health Care
Presently I work in Home & Community Care as a Client Service Coordinator (CSC) at Bayshore Home Health. My main role as a coordinator is to schedule Personal Support Workers (PSWs) and Nurses to individuals in need of services living in the community. This process begins by Case Coordinators from the NE LHIN making assessments based on client referrals. The NE LHIN determines the frequency of visits and the CSC's schedule accordingly. The Ontario Ministry of Health and Long-Term Care (MOHLTC) set the contract with the NE LHIN in order to provide service providers with funding.

"Home care can permit individuals to live at home with independence and dignity, and is part of the solution to some of the challenges facing our health system, such as lengthy wait times for placements and procedures, pressures on emergency departments, inappropriate use of hospital beds, and a shortage of long-term care beds (Canadian HealthCare Association,2009)."

In my role as a CSC I interact with various health professionals in order to provide high quality home care services. Below is a visual display I developed to demonstrate the organizational structure of health care in relation to my role as a coordinator.
